Icons in Appointment Notes

Is it possible to add small icons (or smilies) to the notes section of a patient appointment.

The notes are a useful way of showing infomation specific for the appointment, howeve sometimes to highlight a note would be useful. Using flags does not seem to do the business because that is not visible on the appointment screen unless you open the appointment.
