Clinical outcomes report

Hi I am needing to produce an clinical outcomes based report for my CCG

Can you please advise me how I can produce a report that will show
the numbers of patients to types of outcomes (we have added about 30 clinical outcomes that I need to report on)

Is this possible?

Thanks very much for your post!

I assume the CCG is a custom form that you attach to a patients clinical notes? This certainly is possible i.e. the patient list report that joins to the custom form should work. Would it be possible to have the exact name of the form that you use and we will see if we can work out the SQL for you? As this should be fairly straight forward we can post the report onto the forum for you without an additional charge =D>

thanks for your reply apologies for delay in reply
The outcomes I wish to make the report for are the ones within the patient session notes
drop down box outcomes (we have added about 30 outcomes within this drop down) and need to be able to
show our CCG how many of each outcome was achieved during a quarter
