      For about 2 years now I have been working on Prof orma regional case history recording system and a paediatric CH…

      We have been testing this system for 3 months at my practice in kent and have just made revisions and move to version 2

      The pack consists of an
      Interview sheet
      Regional examination pro forma’s for the cervical and lumbar spine with special tests and neuro screening with visual diagrams to allow for free flowing examination and recording
      Medical Health Questionnaire
      Diagnosis/Treatment and Management Planning
