        Thanks for coming back to us on this. I notice from your screenshot that you are downloading a version 4 report template, but you’re using ClinicOffice version 5. Please download the correct report template for your version of ClinicOffice.

            Please go back to the instructions here : https://pioneersoftware.co.uk/forums/topic/how-do-i-import-a-report-file-into-my-database

            …and download the ClinicOffice v5 templates that you require (not the v4 templates).
